Claim via the hospital
- In case the insured can exercise the right with contractual hospital or clinic as stated in the handbook attached in the policy, the insured shall present P.A. medical card along with ID card or other cards issued by the government to the medical institute when hospitalizes.
- Except In case the insured is being hospitalized in non-contractual hospital or other medical institutes, the insured have to make advance payment and submit documents, original receipt and medical certificate to the Company's address below to claim the expenses.

Period of claim payment
Process of claim takes around 15 days from the day that the Company receives completed and accurate documents.

- The complete claim form and the insured should provide clear details in the description part for an accident.
- Medical certificate which clearly specifies the cause of injury
- Receipts (Original)
- X-ray film or result of X-ray provided in case of broken or crack bones
- Copy of book bank
- Copy of P.A. medical card
- Copy of ID card

- Full length photograph and lost organ photograph of the insured

The beneficiary must submit following documents in case of loss of life
- Copy of autopsy certificate certified by the issuer
- Copy of dissection certified by the issuer (In case of body dissection)
- Copy of death verification letter certified by the issuer
- Copy of police daily report certified by the issuer
- Copy of death certificate certified by the issuer
- Copy of the death person's and beneficiary's ID card and house registration certified by the beneficiary
- Certified copy of ID card
- Certified copy of house registration
- Copy of marriage license of the heir
- Certified copy of the ID card of the Director or partnership who is authorized to act for and on behalf of the company/partnership
- Certified copy of house registration of the authorized person who is authorized to act for and on behalf of the company/partnership
- Copy of certificate of company/partnership registration
- Copy of letter of employment of the dead person
- Medical certificate
- Original or copy or receipt that shows details of the medical expenses
- Copy of book bank
- Copy of P.A. medical card

- The Company's staff will consider claim amount accompanied by aforementioned documents
- Claim will be made to the insured or beneficiary (In case the case is covered by the coverage)
- The Company has the right to request more documents accompanying the claim In case of injury, dismemberment, total permanent disability or death which involve with lawsuit (except road traffic accident case), please attach certified copy of police daily report along with the case result. In case the insured or the covered person changed his/her name-last name, the certificate of name change registration must be submitted .

ทางบริษัทจะดำเนินการแก้ไขข้อมูลภายใน 3 วันทำการ และเอกสารจะส่งให้ลูกค้า จะได้รับเอกสารแก้ไขภายในอาทิตย์ - 2 สัปดาห์เท่านั้น
